Omega Shield LLC – Terms & Conditions

1. Project Description
Omega Shield LLC agrees to provide epoxy floor coating services at the address listed in the form. Services include:
Standard Services Provided:

  • Surface preparation (grinding, shot blasting, vacuuming)

  • Filling and repairing cracks

  • Application of epoxy coating system

Additional Details:

  • Square footage, number of rooms, and any other information provided by the client in the form
     

2. Exclusions
The following items are not included unless specifically stated otherwise:

  • Moving heavy furniture (available for an additional charge)

  • Waterproofing, baseboard work, or any service not listed under “Standard Services Provided”
     

4. Project Timeline

  • Estimated project completion: 1–2 days depending on size and complexity

  • Weather delays will be rescheduled at the discretion of Omega Shield LLC
     

5. Payment Terms
Total Project Cost: Calculated based on the square footage and selected flooring system.
Payment Schedule:

  • 50% deposit due before work begins

  • Remaining balance due upon completion
     

Accepted Payment Methods:

  • Cash

  • Check

  • Debit card

  • Credit card (processing fees may apply)

Late Fees:

  • $50 per month after 30 days past due

  • $100 per month after 60 days past due
     

6. Use of Project Photos
Omega Shield LLC may take photos of the completed project for marketing or portfolio purposes.

7. Dispute Resolution
Any disputes arising from this agreement shall first be addressed through mediation or arbitration prior to either party taking legal action in court.

8. Insurance & Liability
Omega Shield LLC maintains general liability insurance. All reasonable precautions will be taken during the project. Omega Shield LLC is not liable for damages that fall outside the scope of its insurance policy.

9. Entire Agreement
This agreement, together with the submitted form and accepted quote, constitutes the entire agreement between the Client and Omega Shield LLC. Any modifications must be made in writing and signed by both parties.
